Eagle Award

Each year, the NCAHC recognizes both an individual and/or an organization or employer who has demonstrated compelling vision, leadership and excellence in their commitment to improving health outcomes in North Carolina communities.

Pam Seamans

2010 Individual: Pam Seamans, Executive Director, NC Alliance for Health

"Pam is a visionary. She was one of the founding members of the NC Alliance for Health in 2002 and has worked to bring diverse partners together to work collaboratively to achieve strong public health policy change in North Carolina. Her work has literally saved lives and because of her vision and leadership the policy achievements will result in long term healthcare savings for North Carolina. This legislation made North Carolina the first major tobacco-growing state to make all restaurants and bars smoke-free." The American Health Association - American Stroke Association

The mission of the American Heart Association is to build healthier lives, free of cardiovascular disease and stroke. With this being the mission, the AHA has committed vast resources to fight heart disease in North Carolina in the form of both educational campaigns and also research investments. Last year alone, there was over $11 million in research grants awarded to local institutions. Through the Healthier Tomorrow Campaign, over 13,000 students participated in the AHA's Jump Rope for Heart and Hoops for Heart Programs. Through the Get with the Guidelines program, the AHA works with hospitals in the Triangle to ensure they are up to date on the most current information on the treatment of heart disease. With the Know Your Numbers initiative, the AHA has worked with various local community health partners to conduct screenings for warning signs and risk factors for heart disease and stroke. Also, through the CPR Anytime program, the AHA has worked to certify thousands of people on the proper CPR techniques, a critical factor in improving a patients chances of survival in sudden cardiac arrest.

Through all of this and many other initiatives, the number of people in NC dying every year due to heart disease and stroke has been reduced from 30,000 every year as recently as 5 years ago to currently 24,000 per year. While this is a significant accomplishment, the AHA knows there is much more work to be done.

The Poe Center specializes in developing programs for North Carolina's youth to address health concerns such as childhood obesity, physical inactivity, unhealthy food choices, tooth decay, drug and alcohol abuse, unintentional injuries and teen pregnancy.

Our 2009 Winner - 2009: Roanoke Chowan Community Health Center

Roanoke Chowan Community Health Center's (RCCHC) mission is to improve the health status of the underserved and indigent individuals in northeastern NC through enhancing access to quality health care. RCCHC implemented the first Remote Monitoring and Chronic Care Management Program within a Community Health Center site in September 2006. Cardiovascular Disease, Diabetes and Hypertension patients are selected by the patient's primary care provider (PCP) who develops a Plan of Care (POC) based on the individual patient needs. A RN customizes the equipment's software based on the patient's POC, installs the equipment, trains the patient, and daily monitors the patients data, provides follow-up patient health assessment and patient education, and notifies the PCP of abnormal data indicators that may need to be addressed by the PCP. Patients have experienced increased compliance in their medication regimen, medical regimen, and their nutrition regimen. These patients have experienced increased self care, early detection of impending chronic disease exacerbations and high levels of patient satisfaction.

The mission of KDI Health Solutions (KHS) is to optimize healthcare outcomes related to cost, quality, and access, through market leadership in the delivery of quality, innovative, community-based clinical pharmacy services.

Kerr Drug, Inc. (KDI) is a regional chain in North and South Carolina with more than 80 stores in North Carolina and 70% are located in rural areas. For over a decade, KDl's clinical services division has been a leader in delivery of innovative community pharmacist services. Recently, the clinical services division was named KDI Health Solutions (KHS). KHS is a healthcare provider organization with practice sites in Kerr Drug stores. KHS clinical pharmacists are therefore poised to optimize medication use and improve health outcomes for patients in numerous communities throughout North Carolina. Current KHS offerings include delivery of Medication Therapy Management (MTM) services through various state and national programs, a comprehensive Diabetes Education Program (DEP), recognized by the American Diabetes Association, employer based wellness programs including health screenings, individual and group educational interventions, and immunization delivery.

The KHS team has been active in rolling out comprehensive medication evaluation services to the patients in North Carolina and currently delivers MTM services for patients with Humana, Health Spring, Blue Cross Blue Shield and Member Health Medicare Part D plans, as well as for the recipients of the NC Medicaid Focused Outpatient Risk Management (FORM) program.

The Kerr Drug Diabetes Education Program (DEP) is the only community pharmacy based diabetes education program in NC and offers expanded point of access for education, one of the essential components of diabetes care. The Kerr Drug DEP received the American Diabetes Association's 2008 Provider of the Year award in NC.

Additionally, KHS pharmacists administer influenza, pneumococcal, and zoster vaccinations to adults. KHS immunizers improve patients' access by offering vaccinations in community pharmacies, at senior centers, health fairs, and employer sites, which is a benefit to the entire community.
